Cirrhosis, elevated bilirubin, or CrCl < 30 mL/min
Current or recent history of excessive consumption
Active malignancy or recent treatment
Platelets < 100k or on antiplatelet therapy
Prior major bleed
SBP > 160 mmHg currently
Hct < 30% or Hb < 10 g/dL
CYP2C9*2 and/or *3, or other high-risk SNPs
History of falls, gait instability, or frailty
Prior stroke, TIA, or thromboembolism
