When to Use

Assessment Goal

The Clinical Activity Score (CAS) is used to determine if thyroid-associated ophthalmopathy (TAO) is in the active, inflammatory phase where immunosuppression (e.g. steroids, Teprotumumab) is most effective.

Section 2

Formula & Logic

Initial 7 Parameters (1 pt each)

Spontaneous retrobulbar pain
Pain on attempted up or down gaze
Redness of eyelids
Redness of conjunctiva (diffuse)
Swelling of eyelids
Chemosis (swelling of conjunctiva)
Swelling of the caruncle or plica

Interpretation

Active disease is defined as a CAS ≥ 3/7 at the first visit.
