When to Use
When to Use
Prognostic and severity assessment of patients with Chronic Graft-vs-Host Disease (cGvHD).
To guide initiation and tapering of systemic immunosuppression.
Calculating the "Global Severity" for standardized clinical trial reporting.
Patient Population
Patients who have received allogeneic HCT and developed features of chronic GvHD (e.g., sclerodermatous skin changes, sicca syndrome, bronchiolitis obliterans).

Organ Scoring (0–3)
Each organ (Skin, Eyes, Mouth, GI, Liver, Lungs, Joints/Fascia, Genital) is scored from 0 (no involvement) to 3 (severe impairment).
Global Severity Classification

Mild
1 – 2 organs involved (max score 1 in all) AND Lung score 0.
Moderate
≥ 3 organs involved (max score 1) OR At least 1 organ (not lung) score 2 OR Lung score 1.
Severe
At least 1 organ score 3 OR Lung score 2 – 3.

Pearls/Pitfalls
The Lung Priority
In the NIH system, the Lung is prioritized as the most critical organ for global grading. A lung score of 2 (representing significant dyspnea at rest or with minimal effort) immediately upstages the patient to "Severe" regardless of other findings.
QoL Correlation
The NIH Global Severity score correlates strongly with patient-reported quality of life (QoL) and overall mortality, making it a robust endpoint for registrational trials.
